Medical Orders for Life-Sustaining Treatment (MOLST)

Follow the provided directions 1 thru 5 and fax a copy of the certificate to your Regional EMS Program Agency for 1 hour of CME credit.

Take away points:

  • It is distributed as a BRIGHT PINK, cardstock, multi-page form, but it can be photocopied and faxed while remaining a medical order and legally binding.
  • It may replace a NYS non-hospital Do Not Resuscitate (DNR) form.
  • The NYS non hospital DNR form is still valid.
  • It includes information to be used in other health care settings such as the hospital (i.e. placement of feeding tubes, and so on).
